Here at PD Vet Physio I take your privacy and the protection of your data very seriously. As a client your information will only ever be used to assist me in the day to day running of my business and provide you with a first class service.

As a Physiotherapist I will collect personal data such as your name, address, email, details of your animal or your personal history of your health and social life (ie occupation, exercises regime, medical history, medications). This information is collected as part of the assessment process prior to treatment. The information provided is confidential and as such is securely stored and all information is password protected and encrypted and held on a secure server within the UK.

I ask for the information about you/your animal(s) so that you/they can receive the physiotherapy service you requested. I keep this information, together with your details because it may be needed in future if I see you/your animal again and allows continuity of your care. This information will not be shared with a third party without your permission (liaising with vets/saddlers/farriers/insurance companies/GP).  

You have a right to view/amend/remove your/your animals details.  

As a client you have agreed to receive important communication from me via phone/text/email (invoices/receipts/reports/updates). Should you cease to be a client your information will be held for 8 years before it is destroyed. 

 This business will never trade, sell or profit from your personal data.
